   * tta/C/convert/format_html.c (html_convert_index_entry_command_type) (html_convert_def_line_type), tta/perl/Texinfo/Convert/HTML.pm (_convert_index_entry_command_type, _convert_def_line_type): use in_multiple_conversions instead of multi_expanded_region in conditions.  The result is the same, but it makes more sense to avoid outputting a target for an index entry more than once irrespective of the context.
