   (%command_style_map): Remove 'dfn'.
   (@double_quoted_commands, %double_quoted_commands):
   Change from array to hash.
   
   (conversion_initialization): Initialize $self->{'open_double_quote'}
   and $self->{'close_double_quote'}.  Do not copy $self->{'style_map'}
   from %style_map.
