   Comments

   Refine lookup_proto() and test it better.
   Return from the function iff the lookup was successful and remove the
   duplicate error checking from gen_scode().  Handle all failures at the
   end and include the current nominal primitive in the error message.  Add
   comments to explain the context better.  Add a tqkw() helper function.
   
   In the reject tests hard-code only the two Q_DEFAULT cases, but test
   every explicit proto qualifier in the generated cases.  Rename some
   tests to have the name match the filter expression.
